how to apply
Applications are submitted through one of the priority referral sources. Applications can be filled out online and sent to a secure database or can be printed off and faxed to the Unit Registry at Loft Community Services. Once the application has been received, it will be reviewed for basic eligibility criteria and, if eligibility is met, it will be forwarded to one of the four LEAD agencies who will then be in contact to discuss specific details that will be helpful in assessing the client's needs and fit with the agency's resources. Throughout this process we will be in contact to let you know the status of your application. Also, the Unit Registry is available to answer any questions you may have about the program and the application process.
